  • Medfield Parks & Recreation Field & Court Rental Form
  • Field Permits must be submitted to Jim Snyder at 124 North Street, Medfield, MA 02052. In accordance with the process currently in place with other town departments, Medfield Parks & Recreation Dept will forward a copy of your department’s application form along with any information used to advertise the program to be offered to the Board of Health for review. Upon review, the Board of Health Agent will make a determination as to whether or not a camp permit is required and the applicant will be notified in writing. It is necessary to allow at least 90 days for the Board of Health to review a Camp Permit and a determination must be made every year.
  • Summer Camp Information & Health Forms
  • The Parks and Recreation office will need to have an updated immunization and physical records on file before your child’s first day of camp, per Board of Health regulations. We also have a health records form to fill out in case of an emergency during camp. These only need to be filled out once no matter how many weeks of camp your child attends. This will apply to Discovery, Adventure, Teen Xtreme or any sports camps offered this summer.
