The form requires an up-to-date immunization record, physical and emergency contact information in the event of a medical emergency. New this year, all health forms will be completed and stored in an online database through www.campdoc.com. You will only need to complete the form once (should take about 5 minutes) and upload your child's physical and immunization report to the site.
